Os peroneum

Case contributed by Samir Benoudina
Diagnosis almost certain

Presentation

Incidental finding.

Patient Data

Age: 45 years
Gender: Female
x-ray

No evidence of fracture or other osseous pathology.
Two well-corticated small osseous foci noted lateral to cuboid, most probably either a bipartite or fractured os peroneum (given a history of trauma to lateral foot).

Case Discussion

An os peroneum is a small sesamoid bone located just proximal to the base of the fifth metatarsal within the substance of the peroneus longus tendon as it arches around the cuboid. It should not be mistaken for the much rarer os vesalianum, an apophysis, or avulsion fracture of the base of the fifth metatarsal.
