Presentation
Stress-related pain and swelling for several months with p.m. at the proximal lateral tarsus without trauma.
Patient Data
The MRI shows a bone bruise of the os peroneum and surrounding soft tissue edema as well as swelling. The adjoining peroneus longus tendon proximal to the os peroneum is thickened and has high signal in the T2 sequences as well as paratendinous fluid.
Case Discussion
In conclusion, the MRI shows fragmentation of the os peroneum, a partial rupture and tendinopathy of the peroneus longus tendon proximal to the os peroneum and surrounding soft tissue edema. This explains the stress-related pain of the patient and the diagnosis of painful os peroneum syndrome (POPS) was made.
