Presentation

Anterior knee pain.

Patient Data

Age: 14 years
Gender: Male

  • Tendinosis of the lower part of the patellar tendon with edema of the related Hoffa’s pad of fat and related deep infra-patellar bursitis, as well as osseous edema of the anterior aspect of the proximal tibial epiphysis and tibial tuberosity, suggestive of Osgood-Schlatter disease.
  • Mild joint effusion.
  • Intact menisci and cruciate ligaments.

Case Discussion

Osgood-shlatter disease or tibial osteochondritis is a traction apophysitis of the patellar tendon near its insertion within the tibial tuberosity. Osgood-Shlatter disease is a clinical diagnosis and is the most common cause of anterior knee pain in adolescents. The diagnostic hallmark is distal patellar tendinosis with incresed fluid signal around and deep infra-patellar bursitis as well as osseous edema of the tibial tuberosity.
