The infra-patellar ligament appears thickened near its tibial insertion, with abnormal increased signal on proton density fat suppressed images. Mild subjacent marrow edema is seen at the tibial tuberosity and surrounding region of the tibial plateau. Subtle overlying soft tissue swelling and edema is also noted anterior to the tibial tuberosity. Mild edema is also seen involving the inferior angle of the infra-patellar (Hoffa) fat pad.
