History of nephrotic syndrome. Admitted and treated for vomiting and dehydration. Presents with headache, blurred vision, and papilledema.

- triangular-shaped area of T2/FLAIR hyperintense signal involving the pons and spearing the peripheral fibers.
- there is no restricted diffusion and no Gadolinium enhancement
- the cerebral cortex and subcortical white matter are normal
Case Discussion
The clinical presentation and the MRI features, are highly suggestive of osmotic demyelination syndrome.
