Osmotic demyelination syndrome

Diagnosis almost certain

Presentation

History of nephrotic syndrome. Admitted and treated for vomiting and dehydration. Presents with headache, blurred vision, and papilledema.

Patient Data

Age: 15 years
Gender: Female
  • triangular-shaped area of T2/FLAIR hyperintense signal involving the pons and spearing the peripheral fibers.
  • there is no restricted diffusion and no Gadolinium enhancement
  • the cerebral cortex and subcortical white matter are normal

Case Discussion

The clinical presentation and the MRI features, are highly suggestive of osmotic demyelination syndrome.
