Presentation
An athletic patient presenting with chronic pelvic pain, particularly in the left groin.
Patient Data
There is subchondral erosive change, joint irregularity, and sclerosis of the symphysis pubis, consistent with osteitis pubis. Additionally, a bony fragment is observed anterior to the body of the left pubis bone.
There is a linear fluid signal near the left pubis, at the site of rectus abdominis/adductor longus attachment (superior cleft sign).
There is subchondral sclerosis and irregularities.
No significant bone marrow edema of the pubis.
Case Discussion
Considering these features in correlation with the clinical history, the suggestion is athletic pubalgia secondary to osteitis pubis and chronic partial avulsion of the left adductor longus.
