There is a small well-defined osteochondral lesion of the medial talar dome of low signal on T1 and heterogeneous high signal on T2 with bone marrow edema of the medial aspect of the talus and joint effusion in the tibiotalar and subtalar joints.

MRI features most consistent with an osteochondritis dissecans of talus.
