Osteochondroma-related popliteal artery pseudoaneurysm

Presentation

Acute right lower limb pain.

Patient Data

Age: 25 years
Gender: Female

CT angiography of the lower limbs reveals a partially thrombosed popliteal artery pseudoaneurysm caused by a sharp bony exostosis projected from the posterior aspect of the distal portion of the right femur.

An acute thrombus involving a short segment of distally located right popliteal vein is noticed, likely resulting from the mass effect of adjacent pseudoaneurysm

Case Discussion

An acute episode of right limb pain is the main complaint of this young female. Her doctor made suspicion of DVT and the Doppler study of the lower limbs (not shown) reveals acute thrombus in the popliteal vein and a slight decrease in the popliteal artery flow. A hypoechoic lesion was detected in the lower thigh with no apparent flow inside and difficulty tracing cleary-separated vessels. 

The pseudoaneurysm was repaired, and the exostosis was removed. The histopathological study reveals cartilaginous caps and stalks formed of cortical and cancellous bone with no evidence of malignant cells.
