Focal area of sclerosis is noted in the left pedicle of 5th lumbar vertebra.

This is a multi sequential MRI which shows focal area of low T1 and T2 signal intensity related to sclerosis in the left pedicle of L5. There is a central area of high signal intensity within the sclerotic area which shows enhancement post contrast administration. Bone edema is also noted, especially on T2FS sequence.
Case Discussion
Osteoid osteoma is a benign bone lesion that usually present with pain especially at night that relieved by aspirin:
- on CT it is usually seen as a central sclerotic area, with lucency surrounding reactive bone formation
- on MRI the sclerotic area appears as low T1 and T2 signal intensities and the nidus is low on T1, high on T2 with enhancement post contrast administration
