Presentation

History of pain in the left thigh

Patient Data

Age: 10 years
Gender: Male
x-ray

Subtle sclerotic focus in the left femur neck.

mri

Small focus in the left femoral neck just above the greater trochanter showing low T1 and high T2/STIR signal intensities with surrounding marrow edema.
Post-contrast enhancement of the nidus with a hypointense rim in all sequences.
Left side moderate hip joint effusion with synovial thickening.

Case Discussion

Features are of osteoid osteoma with possible inflammatory synovitis. CT scan is the modality of choice for osteoid osteoma. MRI can masquerade a malignant lesion.
