Presentation
Pain with limited range of motion of the left hip.
Patient Data
Small well-circumscribed osteolytic lesion of the femoral neck (nidus) with peripheral reactive sclerosis, focal cortical breach, periosteal reaction and central foci of calcification (central mineralization). Mild joint effusion is noted.
Normal appearance of the right hip.
Case Discussion
CT features most consistent with an osteoid osteoma of the femoral neck.
CT is considered the modality of choice for the detection of osteoid osteoma. It typically shows a focally lucent nidus within the surrounding sclerotic reactive bone. A central sclerotic dot may also be seen.
The most common location is the long bones of the limbs (~65-80%), especially the neck of the femur (as in this case).
