Osteoid osteoma of the sacrum mimicking sacroiliitis

There is a tiny ovoid lesion in the right sacrum inferiorly, which appears iso/hyperintense on T1 and hyperintense on T2 fat sat images. Associated extensive perilesional edema involving the lower half of the right sacroiliac joint.
No SIJ erosion or destruction.
Features most likely represent osteoid osteoma.
Case Discussion
Up to 80% of cases of osteoid osteoma involve the long bones. Location in the sacrum is relatively rare (about 2% of cases). The diagnosis is important to make the appropriate treatment.
