Osteoid osteoma of the talus

Diagnosis almost certain

Presentation

Right foot pain when walking and at rest. Nocturnal pain responding to NSAID treatment.

Patient Data

Age: 10 years
Gender: Male

MRI demonstrates a focally STIR hyperintense, enhancing nidus in the dorsal aspect of the talar neck, surrounded by a rim of low signal due to sclerosis. Note diffuse bone edema and reactive hyperenhancement. Resultant ankle joint of effusion is also present. 

 

Case Discussion

MRI showed an osteoid osteoma of the dorsal talar neck, that was located intra-articularly and lead to arthritis of the ankle joint as well as extensive bone marrow edema around the enhancing nidus.
