Osteoid osteoma - T12 vertebra

Diagnosis almost certain

Presentation

Chonic night lower dorsal pain.

Patient Data

Age: 40 years
Gender: Male

The MRI sequences demonstrate an ill-defined lesion centered of the left pedicle of T12 vertebra of low signal on T1, high signal on T2 with a small central area of low signal well-visualized on T2 and STIR sequences. No enhancement seen on postcontrast sequences.

Few CT cuts were performed after the MRI exam confirm the presence of a small (5 mm) rounded well-circumscribed lucent lesion (nidus) of the left pedicle of T12 vertebra with a central foci of calcification (sclerotic dot) and extensive surounding sclerotic reactive bone

Case Discussion

MRI features are most consistent with an osteoid osteoma of the left pedicle of T12 vertebra.

The nidus, as well as the central mineralization, are better demonstrated on CT than on MRI, however, the bone marrow and soft tissue edema are well-visualized on MRI than CT.
