1-month history of right lateral foot ulcer. Previous history of diabetes mellitus.

Age: 70 years
Gender: Male

Right foot radiographs

There is dressing overlying the fifth digit with a soft tissue defect in keeping with an ulcer. Demineralisation with cortical breaches in the 5th metatarsal head and dorsal dislocation of the 5th metatarsophalangeal joint. Findings are those of osteomyelitis.

Case Discussion

Classic presentation and radiographic appearances of osteomyelitis: a diabetic patient with an ulcer in the foot and x-ray showing regional osteopaenia with bone lysis/cortical loss within the bones underneath the ulcer.
