Osteomyelitis - proximal humerus

Presentation

Right proximal arm - shoulder pain -2 days. Referred for shoulder radiograph and Ultrasound.

Patient Data

Age: about 10 - 12 yrs
Gender: Male
X-ray

Radiograph - No periosteal reaction.

Ultrasound

1st US - Subperiosteal fluid in proximal humeral metaphysis region.
Possibility of acute osteomyelitis suggested in view of clinical background.

Ultrasound

2nd US - 36 hrs after 1st US - Additional finding - Axillary venous thrombus formation.

Case Discussion

Features consistent of  Osteomyelitis. Surgical exploration : Subperiosteal fluid - pus.
Culture report - Staph. aureus
