Bilateral hip pain.
Patient Data
Bilateral loss of sphericity due to collapse of the femoral heads. Large subchondral cysts. Secondary degenerative changes, with subchondral sclerosis of the acetabular roofs and osteophytes formation.
Case Discussion
Osteonecrosis of the femoral head is a relatively common condition. Although many risk factors are known, its pathology is still not completely understood (c. 2024).
In the present case, no risk factor was identified and the case was deemed idiopathic.
The progression of the disease can be staged using the Ficat and Arlet classification. Osteonecrosis of the femoral head with secondary osteoarthritic changes corresponds to stage IV, indicating end-stage disease.
