Age: 6 years
Gender: Male

x-ray

The lateral aspect of the proximal metaphysis of the right tibia shows focal cortical irregularity and sclerosis which warrants further evaluation. 

ct

CT confirms cortical destruction, periosteal reaction, infiltrative and irregular growth of the sclerotic bone lesion,  which are altogether highly suggestive of an aggressive lesion. Exophytic expansion into the adjacent soft tissue is also demonstrated. 

Nuclear medicine

Bone scan demonstrates a focus of marked increased isotope uptake in the proximal right tibia, indicative of increased bone turnover and consistent with the lesion demonstrated on the previous radiographs and CT. No other abnormal focus of abnormal uptake is seen.

mri

Apart from the malignant features also demonstrated by CT, MRI nicely delineates the infiltrative soft tissue expansion and resultant edema best seen on the STIR sequence.
