Lucency at the anterior margin of the left oval window and anterior to the vestibule.  There is subtle patchy lucency in peri-cochlear bone.  

Case Discussion

Appearances consistent with left fenestral and cochlear otosclerosis. Differential for cochlear otosclerosis would include otosyphilis.
