Cochlear implant candidate.
Patient Data



Bilateral extensive fenestral and pericochlear otospongiosis, reaching the anterior IAC walls, with IAC diverticula, more on the right.
Associated left round window stenosis by otospongiosis.
Bilateral stapedial prostheses.



Normal vestibulocochlear nerve bilaterally. No retrocochlear lesion.
Case Discussion
The stapedial protheses are likely to have been done for fenestral (stapedial) otospongiosis, which is the most common form of the disease course (~80%) involving the oval window and the stapes footplate resulting in conductive hearing loss.
As the disease progresses, it affects the cochlea (retrofenestral), which occurs in ~20% of patients with demineralization of the cochlear capsule, resulting in sensorineural hearing loss.
