Age: 45 years
Gender: Female

CT petrous temporal bone study

Axial and coronal temporal bone CT shows a "halo" of radiolucency surrounding both cochleae in keeping with retrofenestral otospongiosis.

There is also radiolucency at the expected location of the fissula ante fenestram on both sides. This represents fenestral otospongiosis.

There is a right stapes prosthesis.

Case Discussion

This case demonstrates bilateral retrofenestral and fenestral otospongiosis.

Otospongiosis begins with bone resorption, followed by fibrosis and vascularization of the temporal bone. It is eventually replaced by dense bone 1. Given most of the clinical course is characterized by lucent bone change, otospongiosis is the preferred term over otosclerosis.
