Ovarian cyst torsion

Presentation

1 day history of sudden onset pain localized to the right iliac fossa. ?appendicitis

Patient Data

Age: 35 years
Gender: Female

CT abdomen pelvis (PV phase)

ct

Large right adnexal mass measuring around 7 cm. Homogenous fluid density (~18 HU) with a thin and regular wall. Normal left ovary. Normal retrocecal appendix.

An intrauterine device is in a lower position at the endometrial cavity.

Incidental bilateral renal pyramidal calcification. Unremarkable appearance of the remaining bowel and solid abdominal organs.

Case Discussion

The patient was taken for laparoscopy where it was found that there was a "...right sided ovarian cyst identified - appeared torsed on itself once, detorted...".

The ovarian cyst was resected and the final histopathology report showed a "theca lutein cyst".

The case highlights how any large adnexal lesion has the propensity to tort on itself and that benign lesions are more likely to tort than malignant lesions.
