Mature cystic ovarian teratoma - floating balls sign

Case contributed by Mohammad Farghali Ali Tosson
Diagnosis almost certain

Presentation

Pelvic swelling.

Patient Data

Age: 30 years
Gender: Female

Well-defined large pelvic cystic lesion with multiple floating balls showing slightly elevated T1 signal, compatible with fat content.

Case Discussion

The appearance of intracystic floating balls is rarely seen but is pathognomonic for mature cystic teratoma 1.

When this typical appearance is found on imaging, the value of other diagnostic tests, such as tumor markers, serological tests for echinococcosis, can be considered as limited.
