Post micturition spot IVU image showing well-defined rounded lucency in the left hemipelvis raising suspicion of dermoid cyst. No obvious calcification is seen within it.

Note the bulky and enlarged left renal shadow and small amount of residual contrast in the right kidney.

Well-defined rounded left ovarian mass containing fat, soft tissue and foci of calcifications within it representing an ovarian dermoid cyst. Severely hydronephrotic left kidney with almost complete loss of renal parenchyma, ballooning of the renal pelvis and an abrupt cut-off at the pelviureteric junction suggestive of pelviureteric junction obstruction.

Case Discussion

Ovarian dermoid cysts are slow-growing tumors contain elements from multiple germ cell layers and are best assessed with ultrasound. Whenever their size exceeds 10 cm or soft tissue plugs and cauliflower appearance with irregular borders are seen, malignant transformation should be suspected.
