Presentation

Incidental left ovarian mass on the ultrasonographic exam.

Patient Data

Age: 40 years
Gender: Female
mri

There is a 34 x 45 mm abnormal signal left adnexa lesion that displays the bright signal on T1 and T2 and suppressed signal on T2 fat suppression sequences denoting fat content and with no evidence of post-contrast enhancement.

Another fluid equivalent signal left adnexa cystic lesion (low on T1 and high on T2 and T2 fat suppression sequences) with a thin smooth margin of enhancement due to a simple ovarian cyst.

Case Discussion

MRI features are consistent with a fat-rich left ovarian lesion with a macroscopic fat signal and signal drop on fat suppression sequences, in keeping with mature cystic ovarian teratoma. Both fat suppression techniques and chemical shift artifacts can be used to confirm the presence of fat.
