Ovarian endometrioma

A multiloculated left ovarian cystic lesion, exhibits high signal intensity on T1-weighted images and low signal intensity on T2-weighted images (shading sign), with fluid-fluid level as well as T2 dark spot sign. No enhancing components in the post-contrast study.
No fatty component is seen in the fat-only images.
Case Discussion
The presence of shading and T2 dark spot signs makes endometrioma more probable than a hemorrhagic cyst.
