Age: 25 years
Gender: Female

The right adnexal cystic lesion measured 37 x 37 mm.

The T1-weighted image shows the characteristic high signal intensity (similar to that of fat) of this right-sided adnexal lesion. However, this signal is not suppressed in the T1 fat saturated image.

The T2-weighted image shows the characteristic low T2-weighted signal. This low T2 signal is a result of the high iron concentration in the endometrioma called (shading sign), which is very suggestive of endometrioma. 

No contrast enhancement was seen with the lesion.

Case Discussion

The patient was fertile, and the gynecologist put her on GnRH for about 6 months and the symptoms (pelvic pain and dysmenorrhea) decreased, but the cyst remains the same size on follow-up ultrasound. The next option is laparoscopic surgery if her symptoms persist.
