Ovarian endometrioma with ipsilateral hemorrhagic cyst - adolescent

Presentation

Dysmenorrhea.

Patient Data

Age: 13 years
Gender: Female

Well-defined left ovarian cyst measuring 45 x 40 x 30 mm of low signal on T1 with an area of high signal, high signal on T2 with a sediment of low signal (hemosiderin products) and peripheral enhancement on postcontrast sequences, in keeping with a hemorrhagic ovarian cyst.

Another small well-defined left ovarian cyst is noted measuring 15 x 13 x 10 mm of high signal on T1/T1 fat sat, low signal T2 with "shading sign" with no peripheral enhancement seen on postcontrast sequences, in keeping with an ovarian endometrioma.

Normal appearance of the uterus and right ovary with numerous small follicles.
