Ovarian endometriomas - shading sign

Diagnosis almost certain

Presentation

Left ovarian lesion on ultrasound.

Patient Data

Age: 35-40 years
Gender: Female

Two well defined cystic lesions at the left ovarian region, the largest one measures 55 x 61 mm and exhibits characteristic T2 shading in the form of a high signal on T1 and low signal on T2. The smaller one measures 40 x 24 mm and exhibits signal of high in T1 and lower high on T2WIs. No soft tissue components. Both lesions are of restricted diffusion.

Right ovary exhibits normal size and harbors multiple small follicles. No abnormal solid or cystic mass. A normal variant of an arcuate uterus with normal uterine contour and myometrium fundal indentation.
