Patient Data
Large well-circumscribed pelvic soft tissue mass of intervesicouterine location arising most likely from the left ovary. It elicits an intermediate signal intensity on T1, low signal intensity on T2 (fibrous component) with central high signal areas representing most likely areas of cystic degeneration +/- edema. Mild heterogeneous enhancement is noted on postcontrast sequences. Small amount of peritoneal fluid.
Normal appearance of the uterus.
Case Discussion
MRI features of an ovarian mass pathologically proven as a fibrothecoma with no tumoral cells within the peritoneal fluid.
Fibrothecomas are considered the most common benign solid ovarian tumor. Usually occur in adult women, with ~66% in postmenopausal women. Larger tumors may be associated with ascites or even Meigs syndrome which may complicate ~1% of cases.
