CT demonstrates two neighboring complex cystic lesions of mixed fat and soft tissue densities seen at the left adnexal region with the larger one shows hyperdense tooth like structure inside. 

Case Discussion

Ovarian mature cystic teratoma is a common ovarian neoplasm in females with rich fat content. A Rokitansky nodule is seen as focal mural thickening, round structure protruding into the cystic lumen, a bridging tissue across the cyst, a cystic structure or sometimes only tooth.  A Rokitansky nodule is noted in this case as round soft tissue nodule and tooth like calcification (within the larger cyst). This appearance is characteristic of ovarian mature cystic teratoma.
