Pelvic pain.

Patient Data

Age: 60 years
Gender: Female

Left adnexal complex cystic lesion is noted, with internal rounded nodule with fat content and a smaller soft tissue mural nodule. The cyst shows also mural calcification and filled with homogeneous hypoattenuating fat-density content.

A left-sided adnexal cystic mass lesion noted showing predominantly both T1 and T2 hyperintense signal with suppression in the fat-saturation sequence (denoting fat content) as well as a small enhancing soft tissue component. These features are in favor of an ovarian dermoid cyst.

Case Discussion

Ovarian mature cystic teratoma is a common ovarian neoplasm with rich fat content and is easily confirmed on MRI. A Rokitansky nodule is seen as a round structure protruding into the cyst lumen. This appearance is characteristic for mature cystic teratoma of the ovary.
