Presentation
Progressive abdominal distension. The ultrasound exam (not shown) revealed a large multiloculated abdominopelvic cystic mass. The tumoral markers (CEA, CA 15.3, CA 19.9, and CA 125) were negative.
Patient Data
The MRI sequences demonstrate a huge abdominopelvic multiloculated cystic mass arising probably from the left ovary. This cystic mass is composed of multiple cysts of various sizes, with variable signal intensity of their content on both T1 and T2 sequences and separated by thin enhanced septa. The small and large bowels are displaced peripherally.
Normal appearance of the right ovary.
Mild intraperitoneal effusion is noted.
No enlarged abdominal or pelvic lymph nodes are seen.
Case Discussion
MRI features of a huge multiloculated cystic ovarian mass with content of variable signal on both T1/T2, thin enhanced septa, and no solid enhanced nodules in a patient with negative tumor markers, suggest an ovarian mucinous cystadenoma which was confirmed after surgery with histopathological exam.
