Ovarian serous adenofibroma

Presentation

Palpable painless pelvic mass on physical exam.

Patient Data

Age: 60 years
Gender: Female

An 80× 45 mm well-defined mass is present in the left pelvis. The mean attenuation value of the mass on non-contrast images is 33 HU, and after contrast media administration, the mean attenuation values increased to 42 HU. The left ovary couldn’t be defined separately from the mentioned mass. There is no local invasion to adjacent structures and no regional lymphadenopathy.

The uterus contains a few fibroids less than 30 mm.

An 18 mm low-enhancing mass is noted in the 6th hepatic segment.

A 7 mm small cyst is also noted in the 3rd hepatic segment.

Case Discussion

The patient underwent a left oophorectomy.

Histopathology:

The sections show the proliferation of spindle cells arranged as fascicles or storiform patterns in a collagenous stroma. A minor component of cystic lesion lined by ciliated cuboidal epithelium is also seen. Features are compatible with left ovarian serous adenofibroma. No malignant changes.
