Presentation

Right lower quadrant pain with a complex right adnexal mass on ultrasound.

Patient Data

Age: 45 years
Gender: Female

Right ovarian unilocular cystic mass of high signal on T1WI/T2WI with a dependent low signal T2WI (intralesional hemorrhage), containing papillary projections. The projections are of intermediate signal on T1WI/T2WI with moderate enhancement and restricted diffusion on DWI/ADC.

The left ovary is normal in appearance.

Mild peritoneal effusion in the pouch of Douglas.

Case Discussion

MRI features of a right ovarian mass with a hemorrhagic cystic component and solid papillary projections which show restricted diffusion, probably representing a serous cystadenocarcinoma.
