Presentation

Abdominal distension.

Age: 65 years

Unilocular cystic mass with homogeneous CT attenuation arising from the right ovary (the ovarian tissue stretched at its posterior and lateral aspect) with a thin regular wall, no endocytic or exocyst vegetation, septae, or enhancing solid component.

The cyst crosses the midline and occupies most of the pelvis and mid-abdomen above the level of the umbilicus, it gently displaces the bowel loops in its vicinity causing no obstruction, and locates immediately under the anterior abdominal wall with no intervening bowels.

Case Discussion

On US, the simple nature of the cyst is ascertained and its appearance is consistent with an ORAD 3 mass, with no peritoneal nodules with features most consistent with a right ovarian serous cystadenoma.

Ovarian serous cystadenoma is the commonest ovarian epithelial neoplasm, usually asymptomatic. On CT it is often unilocular and of homogenous fluid attenuation.
