Ovarian teratoma

Presentation

Abdominal pain and swelling.

Patient Data

Age: 50
Gender: Female

Pelvic MRI

Left adnexal cystic lesion is noted averaging around 11.5 x 18 x 14 cm in maximum AP, CC and side to side dimensions, showing mixed signal intensity of both fat and high proteinacious material, lace-like internal structures of hair. No sign of hemorrhage or infection identified. Its wall measures < 3 mm with no internal septae. The larger fat component along its antro-superior wall shows whorl pattern reflecting hair follicles content. No soft tissue component or enhancing foci identified after contrast analysis. 

The right ovary revealed no adenexal lesions with hemorrhagic corpus luteum cysts. 

RVF uterine body effaced by the adenexal lesion.

Mild Douglas pouch collection.

Case Discussion

The T1 and T2 hyperintense component nilling on fat suppression sequence is consistent with fat content. The T1 iso- and T2 hyperintense signals that is persistent on fat suppression represents high proteinacious material. Lace-like internal structures are representative of hair. The larger fat component along its antro-superior wall shows whorl pattern reflecting hair content.
