The left ovary is larger than normal and exhibits a heterogeneous echo pattern. It is positioned midline, with only peripheral vascularity visible. The right ovary and uterus appear normal.

Case Discussion

Unilateral enlargement of the ovary in a patient with lower abdominal pain, especially with an abnormal midline position, is highly suggestive of ovarian torsion. Due to the dual arterial supply of the ovaries, normal vascularity does not rule out this diagnosis.
