Ovarian torsion

Diagnosis almost certain

Presentation

Lower abdominal pain for 2 days.

Patient Data

Age: 11 years
Gender: Female
ultrasound

US positive findings of ovarian torsion:

  • enlarged right ovary (compared to the left ovary)  with peripherally displaced follicles with hyperechoic central stroma
  • absent right ovarian Doppler flow with normal flow in left ovary
  • enlarged and twisted right vascular pedicle (whirlpool sign)
  • mild free pelvic fluid

Case Discussion

Laparoscopy after a few hours has confirmed an infarcted ovary with clots.
