2nd trimester twin pregnancy presented with severe left iliac pain not relieved by analgesia

Enlarged left ovary (about 5.5 x 8.5 cm) with abundant echogenic stroma and small peripherally displaced follicles, some of them show follicular ring sign 1 (which is a specific sign for ovarian torsion). Doppler scan revealed subtle parenchymal vascularity.
Gravid uterus with two viable fetuses inside. Normal right ovary.
Case Discussion
Left ovarian torsion during pregnancy confirmed by laparoscopy.
No underlying cause was detected.
