Ovarian torsion

Presentation

Three days history of severe pelvic pain.

Patient Data

Age: 30 years
Gender: Female
mri

The right ovary is enlarged and displaced to the left with small peripherally displaced follicles as well as cysts (4 and 5 cm). It elicits an intermediate signal on T1, low signal on T2 with a peripheral rim of high signal on T1 and no enhancement on postcontrast sequences. Twisted pedicle well-demonstrates on T2 sequences.

Minimal effusion is seen in Douglas pouch.

Normal appearance of the left ovary containing small follicles.

Ectopic right kidney.

The clinical presentation and the MRI features are most consistent with an ovarian torsion confirmed at the surgery.
