Acute lower abdominal pain one day duration.
Patient Data
The right ovary is markedly enlarged with high signal intensity at T2 consistent with edema, located posterior to the uterus, contains a thick-walled cyst (corpus luteum) and a simple ovarian cyst measuring 3 x 4 cm, few peripheral located small follicles, Mild streaky enhancement is noticed post-contrast.
The right ovarian pedicle is twisted (Whirlpool sign) as seen at sagittal sequences.
Pelvic free fluid is noted.
Case Discussion
Surgical untwisting can be performed, the ovary was preserved.
Ovarian torsion has a predilection for the right side due to the protective effects of the sigmoid colon on the left.
