Ovarian torsion with dermoid cyst

Presentation

Acute abdominal pain

Patient Data

Age: 15 years
Gender: Female

There is a large well-defined heterogeneous lesion in the right hemipelvis arising from the right ovary returning high, intermediate, and low signal in T1 and T2 weighted images.  The hyperintense parts of the lesion demonstrate loss of signal after saturation of fat signal in T1 and T2 weighted images.  
The right ovary is enlarged in size with T2 hyperintense parenchyma and multiple peripherally located small follicles. It shows no contrast enhancement in post-contrast sequences.  Multifocal T2 low signals suggesting calcifications.
A well-defined cystic structure with internal septations is seen in contact with the left ovary returning high signal in T1 and T2 weighted images. It shows loss of signal after saturation of fat signal in T1 and T2 weighted images.
