Ovarian vein thrombosis

Presentation

3 days postpartum. Lower abdominal pain and diffuse tenderness.

Patient Data

Age: 25 years
Gender: Female

An IUCD is partly embedded in the myometrium. The endometrium is distended, consistent with the recent delivery. Running from the left ovary to the left renal vein, there is a thick-walled structure with blurred outer margins and some central low density. The appearances are consistent with left ovarian vein thrombosis. The right ovarian vein appears normal. There is no bowel abnormality and no perforation.

Case Discussion

In imaging of postpartum patients, ovarian vein thrombosis should be considered and positively excluded. It is seen more commonly on the right side.
