Hypertrophic pachymeningitis characterized by multiple extra-axial vividly enhancing masses involving the cerebral falx and sphenoid wing meninges.

Case Discussion

Her MR images showed diffuse dural based hypointense mass involving the cerebral falx and sphenoid wing meninges. The mass showed dense homogenous enhancement with multiple dural tails. Most probably the etiology in our case is sarcoidosis as we have ruled out the other possibilities like intracranial hypotension, dural fibrosis, metastasis, meningioma, etc.

For a differential please refer to hypertrophic pachymeningitis.
