Radiolucent wedged shaped osteolytic lesions with sharp well-defined borders at the mid-shaft of tibia.

Imaging features can be referred to as blade of grass sign or candle flame sign.

Characteristic appearance of osteolytic or hot phase Paget disease.

In the early phase of Paget disease, there is active bone resorption, often referred to as osteolytic or hot phase.
