Pancreatic adenocarcinoma - staging teaching case

Presentation

Cancer staging.

Patient Data

Gender: Female

Relatively ill-defined hypoenhancing mass in the pancreas head/neck, with abrupt cut off of the dilated pancreas duct leading into it, with upstream atrophy of the pancreas body and tail. Mildly dilated common bile duct, without significant dilation of the gallbladder. Small area of fluid density along the upper aspect of the pancreas head may be a small dilated side branch or amount of peripancreatic fluid. No arterial involvement. Mild narrowing of the main portal vein as it courses along the edge of the tumor, best appreciated on axial images, as well as abutment of the portal splenic confluence. No enlarged peripancreatic lymph nodes. No liver lesions.

Case Discussion

Teaching staging case involving the pancreas head/neck mass with pancreatic ductal obstruction, and only mild dilation of the common bile duct. Only venous involvement without arterial involvement and no metastases.
