Presentation

Acute epigastric pain. Cholelithiasis seen on bedside USS, lipase 1100.

Patient Data

Age: 75 years
Gender: Female
mri

Cholelithiasis with 5 mm calculus within the right hepatic duct. CBD caliber is satisfactory for the patient's age.  No main pancreatic duct dilation. 

Pancreatic divisum noted with the main pancreatic duct draining into the minor papilla superior to the ampulla. The CBD appears to drain normally into the ampulla.

Case Discussion

The patient's mild pancreatitis resolved with conservative management, and was discharged home with a plan for an interval laparoscopic cholecystectomy following medical optimization.
