Pancreatic ductal adenocarcinoma

Diagnosis almost certain

Presentation

Work up for abdominal pain. Elevated CA19-9.

Patient Data

Age: 55 years
Gender: Male

A 60×42 mm low enhancing mass is noted at the pancreatic tail encases splenic vessels.

A few small lymphadenopathy with SAD less than 8 mm are seen in the vicinity of the mass.

A 20×14 mm ill defined hypovascular lesion is noted at the 6th hepatic segment that becomes isodense with surrounding parenchyma on delayed images (ultrasound confirmed hemangioma).

A small amount of fluid is present at perihepatic and perisplenic spaces.

A few stones are apparent in the right kidney.

Pancreatic mass; pathology proved ductal adenocarcinoma with vascular encasement and small regional lymphadenopathy.
