Thyroid nodule follow up.

Patient Data

Age: 30 years
Gender: Female
ultrasound

The right thyroid lobe shows a well-defined heterogeneous, mainly hypoechoic nodule measuring about 1.3 x 1.3 x 1.1 cm with punctate calcification and internal vascularity. Findings are compatible with TI-RADS 5 nodule.

Otherwise, both thyroid lobes and isthmus are normal in echo pattern and size.

Case Discussion

Based on the TI-RAD recommendation, nodules with TI-RAD5 that are larger than 1 cm need a biopsy. Histopathological examination in this case showed papillary carcinoma.

Pathology report:

GROSS: Specimen fixed, consists of 13.5 g thyroid gland. Right lobe: 2.5 x 3 x 1.2 cm, including 1.5 x 1.2 cm whitish solid nodule reaching the anterior surface. Isthmus: 2 x 2 x 1 cm. Left lobe: 2.5 x 3 x 1 cm. Submitted in the same container are few small yellowish fragments, in aggregates 1 x 0.6 cm.

DIAGNOSIS: Thyroid gland, total thyroidectomy: papillary carcinoma, classical type.
